(AP) \u2014 It\u2019s hard to miss the overflowing bowl of condoms at the entrance of the gym.<\/p>\n<p>Some University of Mississippi students walking past after their workout snicker and point, and the few who step forward to consider grabbing a condom rethink it when their friends catch up, laughter trailing behind them. Almost no one actually reaches in to take one. <\/p>\n<p>Though officials say they refill the bowl multiple times a day, and condoms are available at multiple places on campus, Ole Miss students say the disinterest is indicative of changing attitudes. <\/p>\n<p><span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/teen-sex-survey-high-school-3d45d0441f531d1da9f5b44373becee4\">Fewer young people<\/a><\/span> are <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/teen-sex-sexual-intercourse-meaning-fa0b10e0fb7e8fd8fd72ac9990c3321a\">having sex<\/a><\/span>, but the teens and young adults who are sexually active aren\u2019t using condoms as regularly, if at all. And people ages 15 to 24 made up half of new <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/syphilis-gonorrhea-std-sexually-transmitted-cdc-17c748701b8da8024f06869460b33961\">chlamydia, gonorrhea<\/a><\/span> and <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/syphilis-std-cdc-federal-funding-e82f78f3d30c393538e5e421548e7758\">syphilis cases<\/a><\/span> in 2022.<\/p>\n<p>The downward trend in condom usage is due to a few things: medical advancements like <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/iud-pain-lidocaine-gynecologist-f25a198bada157a53514e84dbf57c5a7\">long-term birth control options<\/a><\/span> and <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/doxycycline-std-morning-after-pill-cdc-20a087e32c4a81f7c3413363e6255785\">drugs that prevent sexually transmitted infections<\/a><\/span>; a fading fear of <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/hiv-latino-hispanic-treatment-prevention-aids-ca5f73b064e5ed491ee0315dd60d69d1\">contracting HIV<\/a><\/span>; and <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/florida-sex-education-curriculum-5b1a46f5a73e3122f4815cf1d439aef0\">widely varying degrees of sex education<\/a><\/span> in high schools.<\/p>\n<p>Is this the end of condoms? Not exactly. But it does have some public health experts thinking about how to help younger generations have safe sex, be aware of their options \u2014 condoms included \u2014 and get tested for STIs regularly.<\/p>\n<p>\u201cOld condom ads were meant to scare you, and all of us were scared for the longest time,\u201d said Dr. Joseph Cherabie, medical director of the St. Louis HIV Prevention Training Center. \u201cNow we\u2019re trying to move away from that and focus more on what works for you.\u201d<\/p>\n<div class=\"Enhancement\" data-align-center=\"\">\n<div The fall semester had just started. <\/p>\n<p>Lines of college students with tequila-soda breath waited to be let in dim bars with loud music. Hands wandered, drifting into back pockets of jeans, and they leaned on one another. <\/p>\n<p>It\u2019s likely that many of those students didn\u2019t use a condom, said Magan Perry, president of the college\u2019s Public Health Student Association. <\/p>\n<p>\u201cUsing a condom is just a big, \u2018uh, no,\u2019\u201d the senior said. <\/p>\n<p>Young women often have to initiate using condoms with men, she said, adding that she\u2019s heard of men who tell a sexual partner they\u2019ll just buy emergency contraception the next day instead.<\/p>\n<p>\u201cI\u2019ve had friends who go home with a guy and say they\u2019re not having sex unless they use a condom, and immediately the reaction is either a reluctant, \u2018OK, fine,\u2019 or \u2018If you don\u2019t trust me, then I shouldn\u2019t even be here,\u2019\u201d Perry said. \u201cThey\u2019re like, \u2018Well, I\u2019m not dirty, so why would I use them?\u2019\u201d<\/p>\n<p>Women have long had the onus of preventing pregnancy or STIs, Cherabie said, and buying condoms or emergency contraceptives \u2014 which are often in a locked cabinet or behind a counter \u2014 can be an uncomfortable experience and \u201cinserts a certain amount of shame.\u201d<\/p>\n<p>Annie Loomis, 25, a student at the University of Washington, said dating apps and casual sex are making it hard for people to know what a \u201chealthy sexual relationship\u201d looks like when it comes to intimacy and respect.<\/p>\n<p>\u201cIf you say, \u2018Hey, I want you to wear a condom\u2019 and they say, \u2018no, I don\u2019t,\u2019 you\u2019re not having sex. It should be that simple,\u201d Loomis said. \u201cBut it\u2019s not.\u201d<\/p>\n<p>If pregnancy risk has been the driving factor for condom usage among heterosexual couples, the fear of contracting HIV was the motivation for condom use among men who have sex with men. <\/p>\n<p>But as that fear has subsided, so has condom use, according to <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/link.springer.com\/epdf\/10.1007\/s10461-024-04281-7?sharing_token=csAio9T6IY-zUS2Clm1BT_e4RwlQNchNByi7wbcMAY4CyAozLgMPUnmJuc53uc5x7f5lCIFka2BXa2AcawqHo6uefCodEsNz38dae8-Txl8cJbGqo8ZxNX1oN6Ye5dxUDzuprYTJAoxjP61Jkz1JKpRXDiMJebMSG_dd1YzJums%3D\" target=\"_blank\" rel=\"noopener\">a recent study<\/a><\/span> that focused on a population of HIV-negative men who have sex with men. <\/p>\n<p>Grindr, a popular gay dating app, even lists condom use under \u201ckinks\u201d instead of \u201chealth.\u201d Things like that make Steven Goodreau, an HIV expert at the University of Washington who led the study, worry that the change in attitudes toward condoms is trickling down to younger generations.<\/p>\n<p>Goodreau believes the promotion of pre-exposure prophylaxis (PrEP), a drug that prevents HIV, is overshadowing condoms as a prevention strategy. A <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/www.oar.nih.gov\/sites\/default\/files\/NIH_StrategicPlan_FY2021-2025.pdf\" target=\"_blank\" rel=\"noopener\">strategic plan for federal HIV research through 2025<\/a><\/span> doesn\u2019t mention condoms, and <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/hiv-latino-hispanic-treatment-prevention-aids-ca5f73b064e5ed491ee0315dd60d69d1\">neither does the national Ending the HIV Epidemic plan<\/a><\/span>. <\/p>\n<div (AP Photo\/Karen Pulfer Focht)<\/p>\n<\/figcaption><\/bsp-read-more><\/div>\n<\/p><\/div>\n<\/figure><\/div>\n<\/p><\/div>\n<\/p><\/div>\n<p>The Centers for Disease Control and Prevention acknowledges that condoms are still an effective tool that can be used \u201calongside newer prevention strategies.\u201d<\/p>\n<p>\u201cWe know that condom use has declined among some groups, but they still have an important role to play in STI prevention,\u201d said Dr. Bradley Stoner, director of the CDC\u2019s Division of STD Prevention. \u201cCondoms can be accessed without navigating the health care system, can be used on-demand, are generally affordable and most importantly \u2013 they are effective at preventing HIV and STIs when used consistently and correctly.\u201d<\/p>\n<h2>Medical advances allow for more options<\/h2>\n<p>Pleasure \u2014 for both men and women \u2014 has long been an undeniable factor for the lack of condom use, according to Dr. Cynthia Graham, a member of the Kinsey Institute team that studies condoms. <\/p>\n<p>But more so, <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/africa-aids-hiv-infection-prevention-shot-sunlenca-a94b83717f5b61987822335e11398428\">advances in medicine<\/a><\/span> have expanded the options for both STI and pregnancy prevention.<\/p>\n<p>Young cisgender women have been turning to contraceptive implants like intrauterine devices and birth control pills to keep from getting pregnant. And researchers say that once women are in committed relationships or have one sexual partner for a significant amount of time, they often switch to longer-term birth control methods.<\/p>\n<p>Ole Miss junior Madeline Webb said she and her partner seem like outliers \u2014 they have been seeing each other for four years, but still use condoms. They also share the responsibility of buying condoms. <\/p>\n<p>\u201cPeople see condoms as an inconvenience \u2026 but they do serve a purpose even if you\u2019re on birth control because there is always a chance of an STD,\u201d Webb said. <\/p>\n<p>A new drug on the market could mean even more STI prevention options for men and possibly women. <\/p>\n<p>Doxycycline post-exposure prophylaxis, or doxy PEP, can be taken within 72 hours after unprotected sex and can help prevent chlamydia, gonorrhea and syphilis. It has to be prescribed by a doctor. Trials are still being conducted for women, but the drug is gaining traction among men who have sex with men and transgender women.<\/p>\n<p>With widespread uptake, the drug has the potential to make a significant impact in STI prevention strategies.<\/p>\n<p>\u201cWhen PrEP came out, everyone was excited because it was one less thing to worry about in terms of HIV acquisition,\u201d Cherabie said. \u201cWith another thing on board that can help decrease our likelihood of getting other STIs, on top of not having to worry about HIV, it gives our community and patients a little less anxiety about their sex lives.\u201d<\/p>\n<p>And in just a decade, PrEP has become a main preventive measure against HIV and other STIs for men who have sex with men \u2013 though it is disproportionately used by white men. <\/p>\n<p>Condom use now is \u201cpretty much a thing of the past\u201d for men who have sex with men compared to the 1980s and early 1990s during the AIDS epidemic, said Andres Acosta Ardilla, a community outreach director at an Orlando-based nonprofit primary care clinic that <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/video\/hiv-and-aids-north-carolina-charlotte-d458ee8a29f74eb1b7560e522512287b\">focuses on Latinos with HIV<\/a><\/span>. <\/p>\n<p>\u201cPart of what we have to talk about is that there is something enticing about having condomless sex,\u201d Acosta Ardilla said. \u201cAnd we have to, as people who are working in public Students walked past and grabbed buttons, wristbands and fidget toys. One table offered gold-packaged condoms \u2013 for cups to prevent drinks from being spiked. <\/p>\n<p>Actual condoms are noticeably absent. They\u2019re also absent in the state\u2019s public schools. <\/p>\n<p>Condom demonstrations are banned in Mississippi classrooms, and school districts can provide abstinence-only or \u201cabstinence-plus\u201d sexual education \u2014 both of which can involve discussing condoms and contraceptives.<\/p>\n<p>Focus on the Family, a Christian organization that advocates for teaching abstinence until marriage, is concerned that comprehensive sex education \u201cexposes students to explicit materials.\u201d Abstinence-centered education is \u201cage-appropriate\u201d and keeps students safe and healthy, Focus on the Family analyst Jeff Johnston said in an emailed statement.<\/p>\n<p>But Josh McCawley, deputy director of Teen Health Mississippi, an organization that works with youth to increase access to health resources, said the effects are clear.<\/p>\n<p>\u201cThe obvious consequence is the rise of sexually transmitted infections, which is what we\u2019re seeing right now, which can be a burden on the health care system,\u201d he said, \u201cbut also there could be long-term consequences for young people in terms of thinking about what it means to be healthy and how to protect themselves, and that goes beyond a person\u2019s sexual health.\u201d<\/p>\n<p>The <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/www.cdc.gov\/nchs\/pressroom\/sosmap\/teen-births\/teenbirths.htm\" target=\"_blank\" rel=\"noopener\">latest CDC data from 2022<\/a><\/span> shows Mississippi has the highest teen birth rate in the country. <\/p>\n<p>Scott Clements, who oversees health information for the state education department, was hesitant to criticize Mississippi\u2019s sex education standards because they\u2019re \u201clegislatively mandated.\u201d<\/p>\n<p>\u201cIf the legislature wants to make changes to this, we will certainly follow their lead,\u201d he added \u2014 though attempts to pass more advanced sexual education standards have died repeatedly in the Mississippi statehouse over the past eight years.<\/p>\n<p>Nationally, there is <span class=\"LinkEnhancement\"><a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/sex-education-us-laws-kentucky-florida-mississippi-bd6fffcb3a9a25babc32a143001f4ae1\">no set standard<\/a><\/span> for sex education, according to Michelle Slaybaugh, the director of social impact and strategic communication for the Sexuality Information and Education Council for the United States, which advocates for comprehensive sex ed. <\/p>\n<p>Not every state mandates sex education. Some states emphasize abstinence. Less than half of states require information on contraception. <\/p>\n<p>\u201cThere is no definitive way to describe what sex ed looks like from classroom to classroom, even in the same state, even in the same district,\u201d Slaybaugh said, \u201cbecause it will really be determined by who teaches it.\u201d<\/p>\n<p>Compare Mississippi to Oregon, which has extensive state standards that require all public school districts to teach medically accurate and comprehensive sexual education. Students in Portland are shown how to put on a condom on a wooden model of a penis starting in middle school and have access to free condoms at most high schools. <\/p>\n<p>Lori Kuykendall of Dallas, who helped write abstinence-focused standards, said condom demonstrations like those in Portland \u201cnormalize sexual activity in a classroom full of young people who the majority of are not sexually active.\u201d She also points to increasingly easy access to pornography \u2014 in which people typically do not wear condoms \u2014 is a contributing factor to the decline in condom use among young people.<\/p>\n<p>Jenny Withycombe, the assistant director for health and physical education at Portland Public Schools, acknowledged the standards see pushback in the more conservative and rural parts of Oregon. But the idea is to prepare students for future interactions. <\/p>\n<p>\u201cOur job is to hopefully build the skills so that even if it\u2019s been a while since the (condom) demo &#8230; the person has the skills to go seek out that information, whether it\u2019s from the health center or other reliable and reputable resources,\u201d Withycombe said.<\/p>\n<p>Those standards seem to contribute to a more progressive view of condoms and sex in young adults, said Gavin Leonard, a senior at Reed College in Portland and a former peer advocate for the school\u2019s sexual health and relationship program. <\/p>\n<p>Leonard, who grew up in Memphis \u2013 not far from Oxford, Mississippi, said his peers at Reed may not consistently use condoms, but, in his experience, better understand the consequences of not doing so. They know their options, and they know how to access them. <\/p>\n<p>Slaybaugh wants that level of education for Mississippi students \u2014 and the rest of the country. <\/p>\n<p>\u201cWe would never send a soldier into war without training or the resources they need to keep themselves safe,\u201d she said. \u201cWe would not send them into a battle without a helmet or a bulletproof vest.
