Four people who worked at the plant are still missing, and two have been confirmed dead, The\u00a0<a href=\"https:\/\/apnews.com\/article\/hurricane-helene-tennessee-f7c4a460de588df7a4d5bc9efa756ecb\" target=\"_blank\" rel=\"noopener\">Associated Press<\/a>\u00a0reported. Communities are still plagued by a lack of food, water, power and communications after Helene\u2019s robust wind force and\u00a0powerful floods unleashed over\u00a0<a href=\"https:\/\/www.cnn.com\/2024\/10\/01\/weather\/hurricane-helene-path-of-destruction-climate-dg\/index.html\">500 miles of deadly destruction<\/a>\u00a0from Florida to the Southern Appalachians last week.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wri0sl00083b6md0jkdcuv@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Two state investigations are unfolding into the tragedy as employees, victims\u2019 families and company owners offer differing accounts of the hour before floodwaters overtook the area.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wri0sl00093b6m0rr1lrxk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Families of the victims are outraged because they say employees were made to work during extreme weather conditions, and some were told they couldn\u2019t leave as warnings of heavy rainfall in the flood-prone area poured in.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wri0sl000a3b6mw5smvqia@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Impact Plastics has forcefully denied those claims, saying late Thursday the allegations are false, and no employee was stopped from leaving.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wri0sl000b3b6mh12r4xys@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Here\u2019s what we know about the tragedy and the state investigation:\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrh93f00003b6mbljtyq4q@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            At least two workers at Impact Plastics said they were told to keep working last Friday, just over a mile from a hospital where\u00a0<a href=\"https:\/\/www.cnn.com\/2024\/09\/27\/us\/unicoi-county-hospital-tennessee-flooding-helene\/index.html\">more than 50 people had to be rescued<\/a>\u00a0from the roof due to high floodwaters that same day.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj7ip000m3b6m9a2pd2gw@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Meanwhile, Impact Plastics told CNN a preliminary review of the September 27 flooding event showed all employees had left the plant less than an hour after power went out at the facility and public warning alerts were sent to cell phones.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj7ip000n3b6mgyya6i0a@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Supervisors \u201cdid not prohibit its employees from leaving\u201d and \u201cdid not threaten anyone with discharge from employment,\u201d the company said in the statement on Thursday.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj7ip000o3b6mxrrrr0up@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            El\u00edas Ibarra Mendoza, a 56-year-old grandmother, is one of the two employees who died. Greg Coleman, her family\u2019s attorney, told CNN\u2019s \u201cThe Source with Kaitlin Collins\u201d on Thursday the company\u2019s claims that its management didn\u2019t stop anyone from leaving don\u2019t line up with what he has heard from others.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj7ip000p3b6mor7n8ham@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe problem with that narrative is that\u2019s not what a lot of people are saying or agreeing with. In fact, the exact opposite,\u201d Coleman said. \u201cWe\u2019ve already talked to several that, let\u2019s just say, are opposed to what the company is saying.\u201d\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj7ip000q3b6mid2ywun7@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Senior management was the last to leave about 45 minutes after the plant had been closed and all other employees had been dismissed, the company said.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj7ip000r3b6mqcn7g4o2@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            An employee who made it out of the building safely told\u00a0<a href=\"https:\/\/www.youtube.com\/watch?v=xYfH8nftFpw\" target=\"_blank\" rel=\"noopener\">WCYB<\/a> when he asked if he could leave work after seeing a flooded parking lot, he said he was told no.\u00a0Another employee, Jacob Ingram, told\u00a0<a href=\"https:\/\/www.wvlt.tv\/2024\/10\/01\/they-told-me-no-impact-plastics-employee-says-company-could-have-saved-lives-unicoi-county\/\" target=\"_blank\" rel=\"noopener\">CNN affiliate WVLT<\/a>\u00a0he was told, \u201cNo, not yet\u201d when he asked to leave.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj7ip000s3b6mcmhjcyff@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Sedans and pickup trucks were submerged in brown, murky, fast-moving water as high winds roared in the background, as seen in a video taken by Ingram, who told WVLT he believed lives could have been saved if people left earlier. In one video, more than half of one company building was swallowed by floodwaters.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj7ip000t3b6m4wvaf4m3@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Impact Plastics said its parking lot is in a low-lying area and prone to pooling water, but their review showed water in the lot was about 6 inches deep around the time people were dismissed.\u00a0The \u201cfront of the plant appears to have been passable,\u201d the company said.\n    <\/p>\n<h2 class=\"subheader inline-placeholder\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/cm1wrj9in000v3b6moss9z54n@published\" data-component-name=\"subheader\" id=\"multiple-flood-and-storm-warnings-issued-before-start-of-fatal-shift\" data-article-gutter=\"true\">\n        Multiple flood and storm warnings issued before start of fatal shift<br \/>\n<\/h2>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrj2m5000k3b6mebpci6w9@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            When the first shift at the Impact Plastics factory started at 7 a.m. CT last Friday, the company claimed there was no flooding alert or warning. Water started to pool in the parking lot around 10:35 a.m., the power in the plant went out at 10:39 a.m. and public warnings were sent to cell phones around a minute later, according to the company.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00103b6m3vluoinq@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            That contradicts the multiple flood and storm alerts, along with public warnings from the National Weather Service, that were issued for the area including the Erwin plastics plant prior to the start of the workers\u2019 last shift.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00113b6mvlcv9ypo@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            A flood watch, which included warnings of Helene\u2019s rain, was issued for the area\u00a0<a href=\"https:\/\/mesonet.agron.iastate.edu\/vtec\/#2024-O-NEW-KMRX-FA-A-0007\/USCOMP-N0Q-202409251500\" target=\"_blank\" rel=\"noopener\">on Tuesday<\/a>\u00a0and a flood warning from the National Weather Service was issued at 3:59 p.m. local time on Thursday.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00123b6mcfescanu@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            At 8:14 a.m. local time that Friday, a flash flood warning was issued for Unicoi County, where the factory is located, according to the National Weather Service. By 10:51 a.m., the flash flood warning in the county was updated to a flash flood emergency.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00133b6mdasvx8ur@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            When CNN asked Impact Plastics about the discrepancy, they declined to comment, saying they had \u201cno additional information as the company cooperates with other reviews.\u201d\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00143b6m373p1mek@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cEmployees were directed to leave the plant property within minutes of the power outage and certainly no later than 10:50 AM,\u201d\u00a0which was communicated in both English and Spanish, the company said. Senior management, including its founder, president and CEO Gerald O\u2019Connor, went through the facility to attempt to move the company\u2019s server and important documents, and were the last people to leave around 11:35 a.m., it said.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00153b6m27hciioz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            O\u2019Connor ordered a review the day after the flooding, he said in a Thursday video statement, noting he and the company released the statement because of \u201cdeath threats,\u201d but did not provide additional details on the alleged threats.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00163b6mzzcvgxs3@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            A truck owned by a neighboring company and driven by its employee picked up some factory employees, according to Impact Plastics, but rising water tipped over the truck and \u201cfive employees and a contractor aboard the truck went missing,\u201d the company said in its statement.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00173b6mw5lgg95n@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Five others who were also on the truck when it tipped over made their way to safety and were later evacuated, according to the factory.\u00a0Senior management called emergency responders for help and a National Guard helicopter safely airlifted the five employees, the company said in a statement.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00183b6maifhsv8a@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Employers have eight hours to report a workplace death, TOSHA said,\u00a0<a href=\"https:\/\/www.tn.gov\/workforce\/employees\/safety-health\/tosha-redirect\/fatality--hospitalization-amputation-reporting.html#:~:text=Employers%20are%20required%20by%20law,321%2DOSHA%20(6742).\" target=\"_blank\" rel=\"noopener\">citing Tennessee law<\/a>\u00a0in a Wednesday news release.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta00193b6mxxckrnyr@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The agency, which is working with the state bureau of investigations,\u00a0<a href=\"https:\/\/x.com\/Jobs4_TN\/status\/1841635039760531604\/photo\/1\" target=\"_blank\" rel=\"noopener\">hadn\u2019t yet received a fatality report<\/a>\u00a0from Impact Plastics as of Wednesday evening. It was not immediately clear if there were any workplace deaths, as the company\u2019s founder says nobody perished on company property. The preliminary report from the company also said, to its knowledge, no one was trapped inside the plant or on the premises.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrk5ta001a3b6mxbgv9pwt@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            CNN has reached out to the Unicoi Emergency Management Agency for comment but has not heard back.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrjxp0000y3b6mbaok9ysq@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The Mendoza family is heartbroken as they grieve the loss of Bertha, her son Guillermo Mendoza told CNN on Thursday, as he described his mother as always prioritizing the safety of her grandchildren and children.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrkubi001h3b6mxah1gucu@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The family had just celebrated Mendoza\u2019s 56th birthday last month. Shortly after, another employee texted him and told him the parking lot had flooded, so he went to move his car to higher ground \u2013 there wasn\u2019t a dry spot on the lot, he said.\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrkubj001m3b6m9rarqfbe@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cWe had one way in, one way out,\u201d Jarvis said. \u201cAnd when they told us we could leave, the one way out was blocked off, so we were stuck in traffic on that road, waiting to see what we\u2019re going to do.\u201d\n    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1wrkubj001n3b6m355b6xig@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Now, Jarvis has just one question for Impact Plastics: \u201cWhy\u2019d you make us work that day?
