Where exactly was the cuff? And what specifically was your arm doing?<\/p>\n<p>It\u2019s such a familiar ritual of going to the doctor\u2019s office that most of us scarcely think about the ins and outs, but in reality even small missteps can leave you with an inaccurate reading.<\/p>\n<p>Research has shown that can happen for all kinds of reasons. The cuff size <a href=\"https:\/\/medicine-matters.blogs.hopkinsmedicine.org\/2023\/08\/effects-of-cuff-size-on-the-accuracy-of-blood-pressure-readings-the-cuffsz-randomized-crossover-trial\/\">may be incorrect<\/a>. A patient\u2019s legs <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/10450120\/#:~:text=Conclusions%3A%20Blood%20pressure%20increases%20when,when%20measuring%20their%20blood%20pressure.\">are crossed<\/a> or <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10515304\/\">dangling<\/a> from the exam table. Simply the stress of being in the clinic, what\u2019s known as \u201cwhite coat hypertension,\u201d can <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9634724\/\">spike the numbers<\/a>.<\/p>\n<aside id=\"ad-backstage-wrap\" class=\"ad-wrap backstage\" aria-label=\"advertisement\">\n<\/aside>\n<p>Now add to the list a relatively common faux pas: holding your arm in the wrong position.<\/p>\n<p>A <a href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2824754\">trial from researchers<\/a> at Johns Hopkins University found that simply deviating from the guidelines for arm position can produce an overestimate of a patient\u2019s blood pressure.<\/p>\n<p>The inflated readings might be substantial enough to even lead to a misdiagnosis of hypertension and decisions to start medication, says <a href=\"https:\/\/profiles.hopkinsmedicine.org\/provider\/tammy-mc-loughlin-brady\/2704889\">Dr. Tammy Brady<\/a>, a professor of pediatrics at Johns Hopkins University School of Medicine and senior author of the study, which was published in J<em>AMA Internal Medicine<\/em> on Monday.<\/p>\n<p>\u201cPeople take blood pressure screening for granted,\u201d says Brady, \u201cBut a lot of decisions are made based on what that reading is on the device.\u201d<\/p>\n<p>In the trial, more than 130 adults had their blood pressure measured in three different scenarios, all using an automated device.<\/p>\n<p>The first stuck to the guidelines: Their arm was supported, in this case resting on a desk, with the cuff positioned at about \u201cmid-heart level\u201d on their arm.<\/p>\n<p>The other two, however, strayed from the protocol in ways that Brady sees quite often in clinical practice, with the arm either resting on the patient\u2019s lap or hanging at their side. Both positions produced substantial overestimates of blood pressure.<\/p>\n<aside id=\"ad-secondary-wrap\" class=\"ad-wrap secondary\" aria-label=\"advertisement\">\n<\/aside>\n<p>A patient\u2019s arm resting in their lap could erroneously bump up both the top and bottom number \u2014 known as the systolic and diastolic pressure respectively \u2014 by about 4 millimeters of mercury (mmHg), which is the unit of measurement for blood pressure.<\/p>\n<p>The phenomenon was even more pronounced when the arm was resting at their side. That position increased the systolic by 6.5 mmHg, and the bottom by 4.4 mmHg.<\/p>\n<p>While it may not sound like a huge swing, it\u2019s concerning that a data point used to evaluate risk for cardiovascular disease could be misleading because of such a preventable oversight.<\/p>\n<p>For example, a patient whose systolic blood pressure is 128 mmHg \u2014 which is in the elevated range between 120 and 129 \u2014 a misplaced arm could inadvertently lead them to be classified as solidly in the <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/high-blood-pressure\/in-depth\/blood-pressure\/art-20050982\">hypertensive range,<\/a> which starts at 130.<\/p>\n<p>\u201cThis is very basic, but hopefully reminds everybody it really does matter,\u201d says Brady, whose team has conducted several studies on how blood pressure readings can go awry, \u201cSomebody could be prescribed medication who doesn&#8217;t need it at all.\u201d<\/p>\n<p>Guidelines for measuring blood pressure outline a series of steps for getting the most accurate reading: It\u2019s supposed to be quiet, the patient needs to wait five minutes before the measurement is taken, go to the bathroom ahead of time, keep their feet flat on the floor, their back should be supported, among other requirements.<\/p>\n<p>\u201cEven though we\u2019ve known what to do for a long time, implementing all of this in a real world setting has been tricky,\u201d says <a href=\"https:\/\/www.bidmc.org\/centers-and-departments\/general-medicine-primary-care\/healthcare-associates-hca\/programs-services\/hypertension-center\">Dr. Stephen Juraschek<\/a>, director of the Hypertension Center at Beth Israel Deaconess Medical Center in Boston.<\/p>\n<p>It\u2019s no surprise that some of these details fall by the wayside in the daily rush to see patients, but research also suggests <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5488302\/#:~:text=The%20overall%20performance%20on%20the,%3C20%25%20of%20the%20students.\">new physicians aren\u2019t being trained<\/a> well enough. Another major obstacle is the layout of the exam rooms themselves, which often doesn&#8217;t make it easy for blood pressure to be taken properly. (Even Brady says she has to do some rearranging of the furniture before seeing patients.)<\/p>\n<aside id=\"ad-third-wrap\" class=\"ad-wrap third\" aria-label=\"advertisement\">\n<\/aside>\n<p>Juraschek says this latest research makes it abundantly clear that proper arm position is \u201cessential.\u201d<\/p>\n<p>In his experience, a dangling arm is usually not an isolated problem.
