Suicidal ideation \u2014 and <a href=\"https:\/\/www.huffpost.com\/entry\/help-depression-first-time-support_l_616ee3dfe4b005b245c2d25f\" role=\"link\" class=\" js-entry-link cet-internal-link\" data-vars-item-name=\"the depression that can accompany it\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"616ee3dfe4b005b245c2d25f\" data-vars-target-content-type=\"buzz\" data-vars-type=\"web_internal_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"1\">the depression that can accompany it<\/a> \u2014 often makes people deeply sad or apathetic, which can lead to any number of these scenarios.<\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">But there\u2019s another indicator of suicidal thoughts that may be unexpected or confusing at first: someone with a mental health condition suddenly acting <\/span><em><span style=\"font-weight: 400;\">very<\/span><\/em><span style=\"font-weight: 400;\"> happy or carefree. While this may seem like a good sign, it can actually be concerning.<\/span><\/p>\n<\/div>\n<p><h2><strong>How Can Appearing Happy And Carefree Be A Warning Sign For Suicide?<\/strong><\/h2>\n<\/p>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">Here\u2019s the gist: If someone with a mental health condition suddenly seems joyful and relaxed, it may be because they\u2019ve made the decision to <\/span><a href=\"https:\/\/www.huffpost.com\/entry\/dont-say-committed-suicide_n_60393006c5b6371109dbc63b\" role=\"link\" class=\" js-entry-link cet-internal-link\" data-vars-item-name=\"die by suicide\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"60393006c5b6371109dbc63b\" data-vars-target-content-type=\"buzz\" data-vars-type=\"web_internal_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"2\"><span style=\"font-weight: 400;\">die by suicide<\/span><\/a><span style=\"font-weight: 400;\">. As a result, they feel relieved by two things: the idea that they won\u2019t be in pain much longer, and the fact they can stop debating that decision with themselves. At this point and to this person, dying is the only answer.<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">Ultimately, we have to look at suicide as a health issue, and one that entails a lot of pain, said <\/span><a href=\"https:\/\/afsp.org\/bio\/doreen-marshall-ph-d\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"Doreen Marshall\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/afsp.org\/bio\/doreen-marshall-ph-d\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"3\"><span style=\"font-weight: 400;\">Doreen Marshall<\/span><\/a><span style=\"font-weight: 400;\">, vice president of mission engagement at the American Foundation for Suicide Prevention. <\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">\u201cDespite this pain, those with thoughts of suicide may also be ambivalent about wanting to live or die,\u201d she explained. \u201cThoughts of ending their pain can be met with relief, much like the relief we feel when physical pain is alleviated.\u201d <\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">There\u2019s also another reason that may be at play. \u201cIt is also possible that even if they are struggling with the decision internally, they mask how they really feel inside in an attempt to keep loved ones and mental health providers from finding out about their intention to die,\u201d said <\/span><a href=\"https:\/\/www.gatewaytosolutions.org\/about\/danielle-dellaquila\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"Danielle Dellaquila\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/www.gatewaytosolutions.org\/about\/danielle-dellaquila\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"4\"><span style=\"font-weight: 400;\">Danielle Dellaquila<\/span><\/a>, an<span style=\"font-weight: 400;\"> associate therapist at Gateway to Solutions in New York City. \u201cThis is why it is extremely important that we do not stop assessing for suicide when we see sudden improvement in mental health symptoms.\u201d<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><a href=\"https:\/\/www.huffpost.com\/entry\/how-to-survive-suicidal-ideation_l_630f8682e4b065bfc4b436ec\" role=\"link\" class=\" js-entry-link cet-internal-link\" data-vars-item-name=\"People who are suicidal\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"630f8682e4b065bfc4b436ec\" data-vars-target-content-type=\"buzz\" data-vars-type=\"web_internal_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"5\"><span style=\"font-weight: 400;\">People who are suicidal<\/span><\/a><span style=\"font-weight: 400;\"> are dealing with different brain chemistry that puts them in a dark place with tunnel vision. Plus, that intense level of emotional pain can be blinding. <\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">\u201cFor example, their brain functions differently in terms of judgment, concentration, how they see the world, problem-solving, etc.,\u201d Marshall said. <\/span><\/p>\n<\/div>\n<p><h2><strong>Differentiating Between Suicidality, A Better Day And Mania<\/strong><\/h2>\n<\/p>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">But there\u2019s got to be a line, right? How do we know that a person\u2019s happier attitude is a sign they plan on hurting themselves, versus just that they\u2019re having a better day? Or versus the \u201cups\u201d and energy that mania entails <\/span><a href=\"https:\/\/www.huffpost.com\/entry\/people-bipolar-disorder-want-you-to-know_l_5f204508c5b69fd473124e66\" role=\"link\" class=\" js-entry-link cet-internal-link\" data-vars-item-name=\"in bipolar disorder\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"5f204508c5b69fd473124e66\" data-vars-target-content-type=\"buzz\" data-vars-type=\"web_internal_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"6\"><span style=\"font-weight: 400;\">in bipolar disorder<\/span><\/a><span style=\"font-weight: 400;\">?<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">First, consider the context. \u201cIf the individual\u2019s sudden happy or carefree behavior increases without explanation, that is an indicator to inquire further and not be afraid to ask about the change, as well as to ask directly about thoughts of suicide,\u201d Marshall said. (More on this in a moment.)<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">Next, look for other signs of mania and suicidality, and note how they differ. \u201cManic episodes include other symptoms that would not necessarily be common for a person who is struggling with suicidality to display, such as grandiosity or self-importance,\u201d Dellaquila said. \u201cThose having a manic episode often also seem elated, while an individual who is intent on suicide is more likely to come off as calmer and happier, but not necessarily overjoyed.\u201d <\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">Some other markers of mania<\/span><span style=\"font-weight: 400;\"> include rapid speech, decreased need for sleep and participating in more goal-directed activities, said Dr. <a href=\"https:\/\/www.pathlightbh.com\/clinical-leadership\/howard-weeks\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"Howard Weeks\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/www.pathlightbh.com\/clinical-leadership\/howard-weeks\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"7\">Howard Weeks<\/a>, chief medical officer for Pathlight Mood &amp; Anxiety Center. \u201cMania is also a concerning psychiatric condition and would warrant a referral to local mental health crisis centers,\u201d he added. In other words, getting professional help can be a smart move regardless.<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><a href=\"https:\/\/www.huffingtonpost.co.uk\/entry\/suicide-prevention-warning-signs-how-to-help_uk_6139e26de4b0640100a12790\" role=\"link\" class=\" js-entry-link cet-internal-link\" data-vars-item-name=\"Signs of suicidality\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"6139e26de4b0640100a12790\" data-vars-target-content-type=\"buzz\" data-vars-type=\"web_internal_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"8\"><span style=\"font-weight: 400;\">Signs of suicidality<\/span><\/a><span style=\"font-weight: 400;\">, on the other hand, are <\/span><a href=\"https:\/\/www.huffpost.com\/entry\/what-you-should-do-if-your-child-talks-about-suicide_l_632e126be4b0695c1d8227cb\" role=\"link\" class=\" js-entry-link cet-internal-link\" data-vars-item-name=\"talking about suicide\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"632e126be4b0695c1d8227cb\" data-vars-target-content-type=\"buzz\" data-vars-type=\"web_internal_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"9\"><span style=\"font-weight: 400;\">talking about suicide<\/span><\/a><span style=\"font-weight: 400;\"> (e.g. \u201cI would be better off dead\u201d)<\/span><span style=\"font-weight: 400;\">, seeking lethal means, giving away cherished belongings, and saying goodbye and implying you\u2019ll never see someone again. The signs can also vary from person to person. For example, in <\/span><a href=\"https:\/\/www.huffpost.com\/entry\/depression-symptoms-black-women_l_63b73683e4b0cbfd55e7b6a9\" role=\"link\" class=\" js-entry-link cet-internal-link\" data-vars-item-name=\"Black women with depression\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"63b73683e4b0cbfd55e7b6a9\" data-vars-target-content-type=\"buzz\" data-vars-type=\"web_internal_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"10\"><span style=\"font-weight: 400;\">Black women with depression<\/span><\/a><span style=\"font-weight: 400;\">, you may notice more self-criticism and physical symptoms, such as headaches, low libido and gastrointestinal issues.<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">Week said sudden happiness or relief isn\u2019t a sign you\u2019ll <\/span><em><span style=\"font-weight: 400;\">commonly <\/span><\/em><span style=\"font-weight: 400;\">see, per se, in people experiencing suicidal thoughts. \u201cNothing is ever \u2018exact,\u2019 and many patients who decide to attempt suicide do not experience this switch in mood,\u201d Weeks said. \u201cHowever, it can be a warning sign.\u201d<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">At the same time, the \u201cmasking\u201d element Dellaquila mentioned is very real, too. \u201cThe key thing to remember is many patients do not exhibit classic severe depression symptoms on the surface,\u201d Weeks noted. \u201cThey can be very good at masking their underlying feelings and appear happy and content in discrete social situations.\u201d<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">Here\u2019s the bottom line, according to Marshall: \u201cAny sudden change in the way one talks, their mood or behavior warrants our attention.\u201d<\/span><\/p>\n<\/div>\n<figure class=\"cli cli-image js-no-inject\">\n<div class=\"img-sized\"><picture><source type=\"image\/webp\" srcset=\"https:\/\/img.huffingtonpost.com\/asset\/63ed1d892700003a0009c202.jpeg?ops=scalefit_720_noupscale&amp;format=webp\"\/><img decoding=\"async\" class=\"img-sized__img landscape\" loading=\"lazy\" fetchpriority=\"auto\" alt=\"Asking someone if they have a plan to die isn't going to encourage them -- it might help them get help, instead.\" width=\"720\" height=\"480\" src=\"https:\/\/img.huffingtonpost.com\/asset\/63ed1d892700003a0009c202.jpeg?ops=scalefit_720_noupscale\"\/><\/picture><\/div>\n<div class=\"cli-image__source-wrapper\">\n<div class=\"cli-image__credit\" aria-label=\"Image Credit: Tony Anderson via Getty Images\">\n<p><span class=\"credit\">Tony Anderson via Getty Images<\/span><\/p>\n<\/div><figcaption class=\"cli-image__caption caption-cli\">Asking someone if they have a plan to die isn&#8217;t going to encourage them &#8212; it might help them get help, instead.<\/figcaption><\/div>\n<\/figure>\n<p><h2><strong>How To Help And Support Someone Experiencing Suicidal Thoughts<\/strong><\/h2>\n<\/p>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">You can \u2015 and should \u2015 ask if someone has plans to die by suicide. <\/span><span style=\"font-weight: 400;\">As uncomfortable as that question is to verbalize, it\u2019s crucial. And it won\u2019t \u201cgive the person ideas\u201d like you may (understandably) worry it would, according to <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24998511\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"a study in Psychological Medicine\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24998511\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"11\"><span style=\"font-weight: 400;\">a study in Psychological Medicine<\/span><\/a><span style=\"font-weight: 400;\">. <\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">\u201cOften, they\u2019ll be relieved someone cares enough to hear about their experience with suicidal thoughts,\u201d Marshall explained. <\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">It\u2019s vital to note that the person may not always <\/span><em><span style=\"font-weight: 400;\">say <\/span><\/em><span style=\"font-weight: 400;\">they are planning on hurting themselves, but that doesn\u2019t mean they don\u2019t need support. <\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">\u201cEven if they say no, if you are concerned, you can still encourage them to reach out for help with their own treatment team (therapist, psychiatrist, nurse practitioner, primary care physician) or encourage them to call 988,\u201d Weeks said. (ICYMI, <\/span><a href=\"https:\/\/www.huffpost.com\/entry\/988-mental-health-suicide-hotline_l_62d02b88e4b02074ac934a82\" role=\"link\" class=\" js-entry-link cet-internal-link\" data-vars-item-name=\"988 is the new number for the suicide hotline\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"62d02b88e4b02074ac934a82\" data-vars-target-content-type=\"buzz\" data-vars-type=\"web_internal_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"12\"><span style=\"font-weight: 400;\">988 is the new number for the suicide hotline<\/span><\/a><span style=\"font-weight: 400;\">, which you can call or text.)<\/span><\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p>Other than directly asking, Marshall recommended staying calm, trusting your gut feelings, reminding them you love them, and reassuring them that you hear what they\u2019re saying and aren\u2019t judging or belittling it. She also advised reminding them they\u2019re not alone in what they\u2019re going through, normalizing mental health talk and offering to help connect them with other resources.<\/p>\n<\/div>\n<div class=\"primary-cli cli cli-text \">\n<p><span style=\"font-weight: 400;\">\u201cTake the time to calmly listen to what they have to say, and ask some follow-up questions, like how often they are having suicidal thoughts or what they need to do to feel safe,\u201d she added. \u201cAlso, reassure them that help is available, and that these feelings are a signal that it\u2019s time to talk to a mental health professional.\u201d <\/span><span style=\"font-weight: 400;\">(<\/span><a href=\"https:\/\/openpathcollective.org\/\" role=\"link\" class=\" js-entry-link cet-external-link\" data-vars-item-name=\"Open Path Collective\" data-vars-item-type=\"text\" data-vars-unit-name=\"67080340e4b0f3da64561aad\" data-vars-unit-type=\"buzz_body\" data-vars-target-content-id=\"https:\/\/openpathcollective.org\/\" data-vars-target-content-type=\"url\" data-vars-type=\"web_external_link\" data-vars-subunit-name=\"article_body\" data-vars-subunit-type=\"component\" data-vars-position-in-subunit=\"13\"><span style=\"font-weight: 400;\">Open Path Collective<\/span><\/a><span style=\"font-weight: 400;\"> has a database with more affordable therapists.)
