A recent <a data-i13n=\"cpos:7;pos:1\" href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2820833\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:study;cpos:7;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">study<\/a> even found GLP-1s may reduce the risk of 10 different cancers.<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->The growing list of potential GLP-1 indications suggests the drugs may target the root cause (<a data-i13n=\"cpos:8;pos:1\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10823863\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:inflammation;cpos:8;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">inflammation<\/a>, probably) of the most prevalent and costly conditions in the U.S. If even a fraction of the trials now underway pan out, GLP-1s have the potential to reshape health care as we know it.<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->But they can&#8217;t solve everything. In fact, the GLP-1 phenomenon is making the fragmentation and dysfunction of our health care system even more apparent. Just as GLP-1s may help us discover the common denominator in seemingly disparate diseases, they are shining a bright light on the root causes of the health care system&#8217;s ills.<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->The price tag of GLP-1s in the U.S.\u2014up to $15,000 per year, <a data-i13n=\"cpos:9;pos:1\" href=\"https:\/\/www.healthsystemtracker.org\/brief\/prices-of-drugs-for-weight-loss-in-the-us-and-peer-nations\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:far higher;cpos:9;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">far higher<\/a> than in other affluent countries\u2014has become one of the <a data-i13n=\"cpos:10;pos:1\" href=\"https:\/\/www.statnews.com\/2024\/04\/24\/weight-loss-drugs-to-drive-health-care-spending-reports-show\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:single biggest drivers;cpos:10;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">single biggest drivers<\/a> of rising health care costs. Private employers, already facing an unsustainable cost trend, are feeling the pressure from their workforce to cover the drugs, yet they quite literally may not be able to afford it. Some studies suggest widespread GLP-1 adoption, absent cost controls, could bankrupt Medicare and the <a data-i13n=\"cpos:11;pos:1\" href=\"https:\/\/www.nytimes.com\/2024\/03\/04\/opinion\/ozempic-wegovy-medicare-federal-budget.html\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:health care system as a whole;cpos:11;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">health care system as a whole<\/a>.<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->GLP-1s are also shining a harsh light on the inefficiency and inequity in health care. Those who can afford to pay out of pocket are gobbling up the supply of GLP-1s (in some cases for <a data-i13n=\"cpos:12;pos:1\" href=\"https:\/\/www.cnn.com\/2024\/03\/07\/health\/eli-lilly-oscars-ad\/index.html\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:vanity use;cpos:12;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">vanity use<\/a>), while access remains limited for people on Medicare or Medicaid who are disproportionately burdened by obesity and diabetes. For example, Eli Lilly&#8217;s recent <a data-i13n=\"cpos:13;pos:1\" href=\"https:\/\/finance.yahoo.com\/news\/eli-lilly-slashes-zepbound-prices-for-self-pay-patients-140846671.html\" data-ylk=\"slk:move;cpos:13;pos:1;elm:context_link;itc:0;sec:content-canvas;outcm:mb_qualified_link;_E:mb_qualified_link;ct:story;\" class=\"link  yahoo-link\">move<\/a> to slash the price of Zepbound only applies to patients paying out of pocket; and at several hundred dollars per month, even the markdown price is out of reach for many.<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->Pharmaceutical companies, telehealth providers, and even supplement sellers are marketing GLP-1s directly to consumers to meet the runaway demand. Exploiting a loophole resulting from the GLP-1 shortage, some providers are prescribing <a data-i13n=\"cpos:14;pos:1\" href=\"https:\/\/www.fda.gov\/drugs\/postmarket-drug-safety-information-patients-and-providers\/medications-containing-semaglutide-marketed-type-2-diabetes-or-weight-loss\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:compounded generic versions;cpos:14;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">compounded generic versions<\/a> of the drugs that the FDA has warned <a data-i13n=\"cpos:15;pos:1\" href=\"https:\/\/www.fda.gov\/drugs\/postmarket-drug-safety-information-patients-and-providers\/medications-containing-semaglutide-marketed-type-2-diabetes-or-weight-loss\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:may be unsafe;cpos:15;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">may be unsafe<\/a>.<!-- HTML_TAG_END --><\/p>\n<p><button class=\"secondary-btn fin-size-large readmore-button rounded   yf-15mk0m\" data-ylk=\"elm:readmore;itc:1;sec:content-canvas;slk:Story%20Continues\" aria-label=\"Story Continues\" title=\"Story Continues\"> <span>Story Continues<\/span> <\/button> <\/p>\n<div class=\"read-more-wrapper\" style=\"display: none\">\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->This is a prime example of the limitations of the transactional <a data-i13n=\"cpos:16;pos:1\" href=\"https:\/\/www.statnews.com\/2024\/05\/21\/reports-telehealth-death-greatly-exaggerated\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:Telehealth 1.0 model;cpos:16;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">Telehealth 1.0 model<\/a> and the dangers of consumerism running amok. Patients can easily get compounded GLP-1s, even when lifestyle changes or other approaches are more clinically appropriate. But who is looking after their health once the transaction is complete? Who is helping them manage side effects, as well as their overall physical and mental health?<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->If patients get sick from compounded GLP-1s, they could end up in the ER\u2014and their employer and insurer foot the bill. In this scenario, no one wins.<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->The type of clinicians prescribing GLP-1s has <a data-i13n=\"cpos:17;pos:1\" href=\"https:\/\/www.iqvia.com\/locations\/united-states\/blogs\/2023\/08\/new-demand-in-an-old-market\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:expanded rapidly;cpos:17;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">expanded rapidly<\/a>. In their first act as a diabetes drug, GLP-1s were prescribed almost exclusively by endocrinologists. Now cardiologists, orthopedists, internal medicine physicians, and even psychiatrists are prescribing them\u2014presumably with a different lens than an endocrinologist would, and sometimes without full visibility into the patient&#8217;s overall health. Different specialties are starting to establish their own <a data-i13n=\"cpos:18;pos:1\" href=\"https:\/\/www.endocrinologyadvisor.com\/diabetes\/type-2-diabetes\/american-college-physicians-updates-guideline-type-2-diabetes-treatments\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:clinical guidelines;cpos:18;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">clinical guidelines<\/a> for GLP-1s.<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->Given how siloed specialty care is, it&#8217;s increasingly likely that a primary care physician (PCP) might prescribe GLP-1s for weight management without the patient&#8217;s cardiologist knowing about it\u2014and vice versa. Who&#8217;s looking out for the whole person? Who&#8217;s looking at clinical outcomes and costs in a holistic way\u2014for that patient, and for the system as a whole?<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->I&#8217;m rooting for GLP-1s to be a miracle drug. But the jury is still out, and in the meantime, the GLP-1 frenzy is exposing healthcare stakeholders across the system\u2014patients, employers, insurers, providers\u2014to unsustainable clinical and financial risks.<!-- HTML_TAG_END --><\/p>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->On the plus side, these mounting risks\u2014and the unprecedented attention from consumers and the industry alike\u2014may finally be what it takes to fix broken health care models. And the solutions to the problems surrounding GLP-1s are the same ones we&#8217;ve needed all along:<!-- HTML_TAG_END --><\/p>\n<ul class=\"yf-1woyvo2\">\n<li class=\"yf-1woyvo2\">\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START --><strong>Prevention. <\/strong>The U.S. invests far less in <a data-i13n=\"cpos:19;pos:1\" href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2024\/mar\/finger-on-pulse-primary-care-us-nine-countries\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:preventive and primary care;cpos:19;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">preventive and primary care<\/a> than other affluent nations. Increasing access to primary care and mental health services\u2014including through virtual care\u2014is essential to sustainably address the upstream causes of the conditions we&#8217;re now treating with GLP-1s.<!-- HTML_TAG_END --><\/p>\n<\/li>\n<li class=\"yf-1woyvo2\">\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START --><strong>Integrated care.<\/strong> This includes longitudinal care coordination between PCPs and specialists, as well as navigators and patient advocates. The wrap-around financial and administrative support these care team members provide is especially important given the high cost of the drugs and the challenges of managing chronic conditions like diabetes.<!-- HTML_TAG_END --><\/p>\n<\/li>\n<li class=\"yf-1woyvo2\">\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START --><strong>Outcomes-based payment. <\/strong>The recent push to include GLP-1s in <a data-i13n=\"cpos:20;pos:1\" href=\"https:\/\/kffhealthnews.org\/news\/article\/health-brief-weight-loss-drugs-headed-for-medicare\/\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:Medicare negotiations;cpos:20;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">Medicare negotiations<\/a> is a good start, but it&#8217;s not a silver bullet for the healthcare cost trend. Despite the consumer demand for GLP-1s, <a data-i13n=\"cpos:21;pos:1\" href=\"https:\/\/us.milliman.com\/-\/media\/milliman\/pdfs\/2023-articles\/8-28-23_glp-1s-for-weight-loss_20230824.ashx\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:studies have shown;cpos:21;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">studies have shown<\/a> that as many as two-thirds of patients don&#8217;t stick with the drugs long enough to achieve or sustain the clinical benefits, which means substantial upfront costs with little to no payoff for patients <em>and<\/em> healthcare purchasers. Business and payment models tied to clinical and financial outcomes that matter\u2014and that incentivize judicious prescribing and the integrated care needed to boost adherence and long-term results\u2014are a critical step toward minimizing waste and realizing the full value of GLP-1s.<!-- HTML_TAG_END --><\/p>\n<\/li>\n<\/ul>\n<p class=\"yf-1pe5jgt\"><!-- HTML_TAG_START -->GLP-1s have the potential to transform medicine. But if we continue shoehorning them into our siloed and fragmented health care system, their potential will be stunted.
