Its Falcon 9 launch vehicle, which sent the Crew-8 Dragon into space, was grounded three times between July and September for various malfunctions, <a href=\"https:\/\/gizmodo.com\/nasa-tells-spacex-to-focus-on-safety-after-astronaut-hospitalizations-2000519391\" target=\"_blank\" rel=\"noreferrer noopener\">according to reports.<\/a><\/p>\n<aside class=\"single__inline-module aligncenter wp-block-nypost-editor-primary-tag\">\n<\/aside>\n<p>In July, the partially reusable rocket was grounded after <a href=\"https:\/\/nypost.com\/2024\/07\/12\/business\/spacex-falcon-9-suffers-malfunction-imperiling-starlink-satellite-mission\/\">experiencing an engine malfunction<\/a> that an investigation found was caused by an oxygen leak. It was the first failure in more than seven years for the reliable Falcon 9, which is the world\u2019s most active rocket.<\/p>\n<p>But Falcon 9 launches were then halted in August when a Falcon 9 booster tipped over during a landing and ignited a large blaze. The overworked rocket was back to launching Starlink satellites three days later.<\/p>\n<p>In September, the Falcon 9 rocket launched a crew to the ISS and on its return, missed its targeted splashdown zone because of a suspected fuel leak. The vehicle was then grounded indefinitely and the Federal Aviation Administration said it is requiring an investigation.<\/p>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>A SpaceX Falcon 9 rocket carrying the Dragon spacecraft launched on NASA\u2019s SpaceX Crew-9 mission to the International Space Station on Sept. 28 from Cape Canaveral Space Force Station in Florida.<\/figcaption><\/figure>\n<p>\u201cWhen you look at these recent incidents over the last handful of weeks, it does lead one [to] say that it\u2019s apparent that operating safely requires significant attention to detail as hardware ages and the pace of operations increases,\u201d Rominger said. <\/p>\n<p>\u201cBoth NASA and SpaceX need to maintain focus on safe Crew Dragon operations and not take any \u2018normal\u2019 operations for granted,\u201d he added, noting the \u201cincredibly busy\u201d year SpaceX has had.<\/p>\n<p>When the four astronauts were splashing down in Pensacola, Florida, on Oct. 25, one parachute took longer to unfurl than expected \u2014 but Rominger said that was \u201cwithin the family of expected performance.\u201d<\/p>\n<figure class=\"nyp-slideshow-modal-image wp-block-image aligncenter size-large\"><figcaption>Kent Rominger, former chief of NASA\u2019s Astronaut Corps, warned about prioritizing safety amid a busy schedule of missions and issues with equipment. <span class=\"credit\">PR NEWSWIRE<\/span><\/figcaption><\/figure>\n<p>Elon Musk\u2019s SpaceX has been a NASA\u2019s partner for years, launching nine crews and 44 flights to and from the ISS.
