If that\u2019s the case, it is likely this mutated version of the virus would die out when the teen\u2019s illness resolves. The source of the teen\u2019s infection has not been determined, so it\u2019s impossible to know for sure if the mutations were in the virus that infected him or her. But scientists think it is more likely that the mutations developed during the course of his or her infection.<\/p>\n<p>Still, the fact that the mutations occurred at all is a reminder that H5N1 is a dangerous virus for people, one that could potentially trigger a pandemic if it acquired the capacity to easily infect people, flu virologists say.<\/p>\n<p>\u201cBy no means is this Day 1 of a pandemic. There\u2019s no indication \u2026 of human-to-human spread, which is all good. But this is exactly the scenario that we fear,\u201d Scott Hensley, a professor of microbiology at the University of Pennsylvania\u2019s Perelman School of Medicine, told STAT in an interview.<\/p>\n<p>Hensley sparked some concern on social media platforms over the weekend when he remarked on the fact that the genetic sequence of the virus showed key mutational changes in the hemagglutinin, a protein on the virus\u2019 surface that attaches to cells the virus is trying to invade. The sequence data were posted to open-access databases by the Public Health Agency of Canada.\u00a0<\/p>\n<aside class=\"wp-block-stat-article-recirc-single\">\n<div class=\"recirc-single-image\" data-href=\"https:\/\/www.statnews.com\/2024\/11\/13\/bird-flu-canada-teenager-infected-different-strain-than-dairy-cattle\/\">\n\t\t\t<img decoding=\"async\" width=\"768\" height=\"432\" src=\"https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/06\/53289634317_9b59448ee7_3k-768x432.jpg\" class=\"attachment-article-main-medium-large size-article-main-medium-large wp-post-image\" alt=\"\" loading=\"lazy\" sizes=\"auto, (min-width: 540px) and (max-width: 767px) 288px, (min-width: 768px) and (max-width: 1023px) 257px, (min-width: 1024px) and (max-width: 1199px) 287px, (min-width: 1200px) 245px, 246px\" srcset=\"https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/06\/53289634317_9b59448ee7_3k-768x432.jpg 768w, https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/06\/53289634317_9b59448ee7_3k-1024x576.jpg 1024w, https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/06\/53289634317_9b59448ee7_3k-2048x1152.jpg 2048w, https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/06\/53289634317_9b59448ee7_3k-1600x900.jpg 1600w\"\/>\t\t<\/div>\n<h3 class=\"recirc-single-heading\">\n\t\t\t\t\t\t<a href=\"https:\/\/www.statnews.com\/2024\/11\/13\/bird-flu-canada-teenager-infected-different-strain-than-dairy-cattle\/\" rel=\"\">Canadian teen\u2019s bird flu infection is not the version found in cows<\/a><br \/>\n\t\t<\/h3>\n<\/aside>\n<p>\u201cThis is bad news,\u201d Hensley said in a <a href=\"https:\/\/bsky.app\/profile\/scottehensley.bsky.social\/post\/3lb36uy5a7k25\" target=\"_blank\" rel=\"noopener\">Bluesky post<\/a> on Saturday. \u201cWe need to closely monitor this situation and increase our surveillance efforts.\u201d<\/p>\n<p>H5N1\u2019s hemagglutinin preferentially binds to cells with receptors known as alpha 2-3, which are abundant in wild birds and domestic poultry, but are also found in the conjunctiva, the tissue surrounding human eyes, and in human upper airways. The cell receptors in human lungs are known as alpha 2-6. It\u2019s thought that to become a virus capable of spreading easily among people, H5N1 would need to acquire the ability to attach to this latter type of receptor.<\/p>\n<p>Two mutations spotted in the Canadian teen\u2019s virus are known to help flu viruses make this attachment switch. \u201cBoth these sites play an important role in \u2026 binding specificity,\u201d Jesse Bloom, an evolutionary virologist at the Fred Hutchinson Cancer Center in Seattle, said in a <a href=\"https:\/\/bsky.app\/profile\/jbloomlab.bsky.social\/post\/3lb3avexjhk24\" target=\"_blank\" rel=\"noopener\">series of posts<\/a> responding to Hensley.\u00a0<\/p>\n<p>Hensley agreed. \u201cTen out of 10 flu virologists will tell you that these substitutions are important for effective receptor specificity. There\u2019s no question about that.\u201d<\/p>\n<p>The British Columbia case is garnering substantial attention for two reasons. Firstly, how the teenager became infected remains a mystery. Secondly, while H5N1 cases have historically been seen to cause severe disease \u2014 and death \u2014 in a substantial number of cases, the versions of the virus currently circulating in North America have triggered only mild infections except for in this case.\u00a0<\/p>\n<aside class=\"wp-block-stat-article-recirc-single\">\n<div class=\"recirc-single-image\" data-href=\"https:\/\/www.statnews.com\/2024\/11\/07\/bird-flu-farm-worker-infections-undetected-cdc-study\/\">\n\t\t\t<img decoding=\"async\" width=\"768\" height=\"432\" src=\"https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/11\/AP22263597743932-768x432.jpg\" class=\"attachment-article-main-medium-large size-article-main-medium-large wp-post-image\" alt=\"\" loading=\"lazy\" sizes=\"auto, (min-width: 540px) and (max-width: 767px) 288px, (min-width: 768px) and (max-width: 1023px) 257px, (min-width: 1024px) and (max-width: 1199px) 287px, (min-width: 1200px) 245px, 246px\" srcset=\"https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/11\/AP22263597743932-768x432.jpg 768w, https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/11\/AP22263597743932-1024x576.jpg 1024w, https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/11\/AP22263597743932-2048x1152.jpg 2048w, https:\/\/www.statnews.com\/wp-content\/uploads\/2024\/11\/AP22263597743932-1600x900.jpg 1600w\"\/>\t\t<\/div>\n<h3 class=\"recirc-single-heading\">\n\t\t\t\t\t\t<a href=\"https:\/\/www.statnews.com\/2024\/11\/07\/bird-flu-farm-worker-infections-undetected-cdc-study\/\" rel=\"\">Bird flu infections in farmworkers are going undetected, study shows<\/a><br \/>\n\t\t<\/h3>\n<\/aside>\n<p>As of Monday, the United States had reported <a href=\"https:\/\/www.cdc.gov\/bird-flu\/situation-summary\/index.html\" target=\"_blank\" rel=\"noopener\">53 confirmed infections<\/a> this year, all <a href=\"https:\/\/www.statnews.com\/2024\/09\/12\/h5-bird-flu-human-infection-missouri-cause-remains-unknown\/\">but one<\/a> of which were in dairy farmworkers or people involved in culling infected poultry operations. All of these people experienced mild illness, mostly conjunctivitis \u2014 pink eye \u2014 and in some cases minor respiratory symptoms. None of the people infected in the U.S. was ill enough to require hospitalization.<\/p>\n<p>The British Columbia teen was admitted to hospital on Nov. 8 and remains in critical condition there, Provincial Health Officer Bonnie Henry said in an email Monday. It does appear that the individual did not transmit the infection to anyone else.<\/p>\n<p>\u201cAll contacts [are] \u2026 now beyond 10 days post exposure,\u201d Henry said, which would put people who were in contact with the teen before the illness was identified and transmission precautions were put in place outside of the incubation period for flu.<\/p>\n<p>The version of the virus that infected the teenager is not the one circulating in dairy cattle in the U.S. Both are from a strain of H5N1 known as 2.3.4.4b. But the cow virus is from a subset of viruses known as genotype B3.13. The teen was infected with a virus from the D1.1 genotype \u2014 a version of the virus circulating in wild birds. It is the version of the virus responsible for a number of poultry operation outbreaks, both in British Columbia and in Washington state, where 11 confirmed and three probable cases of H5N1 infection have been detected in the past month or so.<\/p>\n<p>Richard Webby, a flu virologist and director of the World Health Organization\u2019s Collaborating Center for Studies on the Ecology of Influenza in Animals, located at St. Jude Children\u2019s Research Hospital in Memphis, Tenn., said he finds the D1.1 viruses particularly concerning.<\/p>\n<p>Influenza A viruses have the capacity to swap genes, and this version of H5N1 has picked up a different neuraminidase gene (the N in a flu virus\u2019 name) than the B3.13 viruses have, Webby said. It\u2019s about to get harder<\/a><br \/>\n\t\t<\/h3>\n<\/aside>\n<p>\u201cJust because it has picked up the new [neuraminidase] doesn\u2019t necessarily mean the [hemagglutinin] will change,\u201d Webby said in an email. \u201cBut the old combination was clearly well matched. The new [neuraminidase] could change that balance and help foster changes, but that is a <em>could<\/em> rather than a <em>will<\/em>.\u201d<\/p>\n<p>Hensley said the evidence of the virus changing in the teenager should serve as a reminder that the mild cases the U.S. has recorded this year may not reflect how the virus might behave if it were to start to spread in people. He is unsettled by the sheer volume of the virus in the environment, and the daily exposure of people to it, on infected dairy or poultry farms.<\/p>\n<p>\u201cYou have a lot of potential human exposures and then what you fear is either a random substitution that emerges during that infection that gives the virus a favorable profile for infecting \u2026 transmitting among humans, and then that takes off.
