Image credit: Grace Cary\/Getty Images.<\/figcaption><\/figure>\n<\/div>\n<\/div>\n<div>\n<ul>\n<li><strong>The number of people with dementia is forecast to almost triple by 2050.<\/strong><\/li>\n<li><strong>Around 70% of these cases are likely to be Alzheimer\u2019s disease, the most common form of dementia.<\/strong><\/li>\n<li><strong>Current treatments can relieve some of the symptoms, and new disease-modifying treatments are not widely available.<\/strong><\/li>\n<li><strong>Now, researchers have developed a nasal spray that, in a mouse model, slows down inflammation and clears protein buildup in Alzheimer\u2019s disease.<\/strong><\/li>\n<li><strong>The researchers suggest the spray might delay Alzheimer\u2019s progression by up to 15 years in people.<\/strong><\/li>\n<\/ul>\n<\/div>\n<div>\n<p>Population growth and aging mean that the number of people with dementia is forecast to reach <hl-trusted-source source=\"The Lancet\" rationale=\"Highly respected journal,Expert written journal,Peer reviewed journal\"><a href=\"https:\/\/www.thelancet.com\/journals\/lanpub\/article\/PIIS2468-2667(21)00249-8\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-90fpmc\">almost 152.8 million<\/a><\/hl-trusted-source> by 2050.<\/p>\n<\/div>\n<div>\n<p>They clear the <hl-trusted-source source=\"National Institute on Aging\" rationale=\"Governmental authority\"><a href=\"https:\/\/www.nia.nih.gov\/health\/alzheimers-causes-and-risk-factors\/what-happens-brain-alzheimers-disease#:~:text=Amyloid%20plaques&amp;text=It%20comes%20in%20several%20different,plaques%20that%20disrupt%20cell%20function.\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-90fpmc\">beta-amyloid plaques<\/a><\/hl-trusted-source> that are a characteristic of Alzheimer\u2019s, potentially delaying the cognitive symptoms. However, the treatments are expensive, and some <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/alzheimers-treatments-the-risk-of-monoclonal-antibodies-may-outweigh-benefits\" class=\"content-link css-90fpmc\">experts are concerned<\/a> that the risk of side effects may outweigh their benefits.<\/p>\n<\/div>\n<div>\n<p>In a new study from Texas A &amp; M University College of Medicine, researchers have used a nasal spray to target microglia and astrocytes \u2014 cells that cause neuroinflammation (brain inflammation) \u2014 delaying the progression of Alzheimer\u2019s disease in a mouse model.<\/p>\n<\/div>\n<div>\n<p><strong>They suggest that, if similar effects are confirmed in people, the spray could delay Alzheimer\u2019s progression by up to 15 years.<\/strong><\/p>\n<\/div>\n<div>\n<p><a href=\"https:\/\/www.alz.org\/press\/spokespeople\/courtney-kloske\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-90fpmc\">Courtney Kloske, PhD<\/a>, Alzheimer\u2019s Association director of scientific engagement, who was not involved in this study, told <em>Medical News Today<\/em> that its findings are encouraging but that much more research is needed to confirm them:<\/p>\n<\/div>\n<div>\n<blockquote class=\"is-layout-flow wp-block-quote-is-layout-flow css-34gtoi\">\n<p>\u201cModels are important in helping us understand the basic biology of the disease, but we need human studies in representative populations for ideas to be fully validated. Microglia are an incredibly complex immune cell in the brain and researchers are still working to understand why they respond the way they do at different points in disease. Therefore, while these are intriguing findings, more research is needed to understand the impacts and outcomes of this kind of intervention on people living with, or at risk for, Alzheimer\u2019s.\u201d<\/p>\n<\/blockquote>\n<\/div>\n<\/div>\n<div><span style=\"font-size:0;line-height:0\"\/><span style=\"font-size:0;line-height:0\"\/><\/p>\n<div>\n<p>Microglia and astrocytes play a key role in neuroinflammation in Alzheimer\u2019s disease. In healthy brains, they <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8167938\/#:~:text=Microglia%20are%20resident%20cells%20of,neuronal%20networks%2C%20and%20injury%20repair.\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-90fpmc\">protect nerve cells<\/a> and remove damaged nerve tissue, but in Alzheimer\u2019s, after initially clearing beta-amyloid plaques, they become overactive and destroy nerve cells.<\/p>\n<\/div>\n<div>\n<p><strong>Using a mouse model of the early stages of Alzheimer\u2019s disease, the researchers administered a nasal spray containing an anti-inflammatory treatment derived from stem cells in <hl-trusted-source source=\"Nature\" rationale=\"Highly respected journal,Expert written journal,Peer reviewed journal\"><a href=\"https:\/\/www.nature.com\/articles\/s41565-021-00931-2\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-90fpmc\">extracellular vesicles<\/a><\/hl-trusted-source>.<\/strong><\/p>\n<\/div>\n<div>\n<p>The aim was to target these immune cells to decrease inflammation and reduce the buildup of harmful proteins in the brain.<\/p>\n<\/div>\n<div>\n<p>They gave the 3-month old mice \u2014 both mice genetically modified to display Alzheimer\u2019s-like symptoms (transgenic mice) and wild-type mice \u2014 2 doses of the nasal spray containing the treatment, or a placebo spray, 1 week apart.<\/p>\n<\/div>\n<div>\n<p>Seventy-two hours after the second dose, they euthanized five mice, to assess the numbers and activity of microglia and astrocytes. <\/p>\n<\/div>\n<div>\n<p>Three weeks after the second treatment, they subjected the rest of the mice to a series of behavioral tests. The researchers repeated these tests regularly over the next month to monitor the mice\u2019s cognitive function following treatment. They then euthanized the mice and analyzed their brains.<\/p>\n<\/div>\n<\/div>\n<div><span style=\"font-size:0;line-height:0\"\/><span style=\"font-size:0;line-height:0\"\/><\/p>\n<div>\n<p>In this mouse model, untreated transgenic mice usually show characteristic signs of Alzheimer\u2019s such as beta-amyloid plaques, increased microglial activity, and inflammation by the age of 4.5 months. <\/p>\n<\/div>\n<div>\n<p>However, at 4.5 months old, the mice that received the nasal spray treatment in this study had reduced microglial clusters, as well as reduced activation of genes associated with neuroinflammation. In addition, they had fewer beta-amyloid plaques than the untreated mice.<\/p>\n<\/div>\n<div>\n<p><strong>These reduced inflammatory effects were most notable in the <\/strong><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3548359\/\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-90fpmc\"><strong>hippocampus<\/strong><\/a><strong> \u2014 the area of the brain that plays a major role in learning and memory \u2014 which is severely affected by Alzheimer\u2019s disease.<\/strong><\/p>\n<\/div>\n<div>\n<p>In behavioral tests, both male and female treated mice showed better cognitive and mood function than the untreated mice. <\/p>\n<\/div>\n<div>\n<p>However, <a href=\"https:\/\/www.providence.org\/doctors\/neurology\/ca\/santa-monica\/clifford-segil-1457556581\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-90fpmc\">Clifford Segil, DO<\/a>, a neurologist at Providence Saint John\u2019s Health Center in Santa Monica, CA, who was not involved in this research, emphasized that this was early days for plaque-removing treatments.<\/p>\n<\/div>\n<div>\n<p>\u201cThe authors in the study noted nasal delivered stem cells could decrease the number of plaques in Alzheimer\u2019s dementia patients and there are medications being used around the world right now doing the same thing. Post-marketing surveillance is going to determine if these plaque reducing medicines cause any noticeable cognitive improvements,\u201d he told <em>MNT<\/em>.<\/p>\n<\/div>\n<div>\n<p>\u201cIf the clinical use results in patients with improved memory, unlike the trials which resulted in these medications\u2019 approval, novel and early methods to decrease brain plaques will be extremely desirable,\u201d Segil added.<\/p>\n<\/div>\n<p><span class=\"css-zya54r\"\/><\/div>\n<div><span style=\"font-size:0;line-height:0\"\/><span style=\"font-size:0;line-height:0\"\/><\/p>\n<div>\n<p>Reducing the activity of microglia could lead to a reduction in their beneficial effects, but this was not seen when mice were treated with the nasal spray.<\/p>\n<\/div>\n<div>\n<p>In a <a href=\"https:\/\/scitechdaily.com\/scientists-develop-breakthrough-nasal-spray-that-could-delay-alzheimers-by-over-a-decade\/\" target=\"_blank\" rel=\"noopener noreferrer\" class=\"content-link css-90fpmc\">press release<\/a>, the study authors said that \u201can intake of neural stem cell-derived extracellular vesicles significantly changed microglia gene expression and reduced the multiple harmful proinflammatory proteins without affecting the microglia\u2019s ability to continue clearing the protein buildup related to Alzheimer\u2019s.\u201d<\/p>\n<\/div>\n<div>\n<blockquote class=\"is-layout-flow wp-block-quote-is-layout-flow css-34gtoi\">\n<p>\u201cThe nasal spray appears to regulate microglia activity effectively. By preventing overactivation of microglia, it reduces harmful inflammation while allowing these cells to continue clearing beta-amyloid plaques, which are associated with Alzheimer\u2019s progression. This balance is crucial because excessive inflammation can lead to neuron damage, while clearing plaques is necessary to maintain brain health.\u201d<\/p>\n<\/blockquote>\n<\/div>\n<div>\n<p>However, he warned <em>MNT<\/em> of potential side effects. \u201cWhile the study shows promising results, possible side effects need to be evaluated,\u201d he cautioned.<\/p>\n<\/div>\n<div>\n<p><strong>\u201cAdverse reactions could arise from altering immune cell behaviour, unexpected impacts on other cell types, or long-term consequences of manipulating the brain\u2019s immune response. Clinical trials would need to monitor any immune-related side effects or unexpected impacts on cognition,\u201d Allder further explained.<\/strong><\/p>\n<\/div>\n<\/div>\n<div><span style=\"font-size:0;line-height:0\"\/><span style=\"font-size:0;line-height:0\"\/><\/p>\n<div>\n<p>Kloske stressed the need for further research to increase the range of Alzheimer\u2019s treatments available.<\/p>\n<\/div>\n<div>\n<p>\u201cTreatments that target Alzheimer\u2019s from all angles and all stages of the disease are essential, and that\u2019s why strategic research funding that works to diversify the therapies in the pipeline is so important,\u201c she told us.<\/p>\n<\/div>\n<div>\n<p><strong>\u201cIt is also important to consider different mechanisms of drug delivery, such as intranasal delivery as is used in this research study.
