But the specific pathways by which the virus does so are still being elucidated, and curative treatments are nonexistent.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Now, two 2024 studies published in the New England Journal of Medicine shed further light on the <a href=\"https:\/\/doi.org\/10.1056\/NEJMe2400189\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:profound toll of COVID-19 on cognitive health;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">profound toll of COVID-19 on cognitive health<\/a>.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">I am a <a href=\"https:\/\/scholar.google.com\/citations?user=DtuRVcUAAAAJ\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:physician scientist;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">physician scientist<\/a>, and I have been devoted to studying <a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/long-term-effects\/index.html\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:long COVID;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">long COVID<\/a> since early patient reports about this condition \u2013 even before the term \u201clong COVID\u201d was coined. I have testified before the U.S. Senate as <a href=\"https:\/\/www.help.senate.gov\/hearings\/addressing-long-covid-advancing-research-and-improving-patient-care\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:an expert witness on long COVID;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">an expert witness on long COVID<\/a> and have <a href=\"https:\/\/scholar.google.com\/citations?view_op=search_authors&amp;hl=en&amp;mauthors=label:long_covid\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:published extensively;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">published extensively<\/a> on this topic.<\/p>\n<h2 class=\"col-body mb-4 text-xl font-bold md:text-2xl\">How COVID-19 leaves its mark on the brain<\/h2>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Here are some of the most important studies to date documenting how COVID-19 affects brain health:<\/p>\n<ul class=\"col-body mb-4\">\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Large epidemiological analyses showed that people who had COVID-19 were at an <a href=\"https:\/\/doi.org\/10.1038\/s41591-022-02001-z\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:increased risk of cognitive deficits;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">increased risk of cognitive deficits<\/a>, such as memory problems.<\/p>\n<\/div>\n<\/li>\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Imaging studies done in people before and after their COVID-19 infections show <a href=\"https:\/\/doi.org\/10.1038\/s41586-022-04569-5\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:shrinkage of brain volume;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">shrinkage of brain volume<\/a> and <a href=\"https:\/\/theconversation.com\/even-mild-cases-of-covid-19-can-leave-a-mark-on-the-brain-such-as-reductions-in-gray-matter-a-neuroscientist-explains-emerging-research-178499\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:altered brain structure after infection;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">altered brain structure after infection<\/a>.<\/p>\n<\/div>\n<\/li>\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">A study of people with mild to moderate COVID-19 showed significant prolonged inflammation of the brain and <a href=\"https:\/\/doi.org\/10.1073\/pnas.2217232120\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:changes that are commensurate with seven years of brain aging;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">changes that are commensurate with seven years of brain aging<\/a>.<\/p>\n<\/div>\n<\/li>\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Severe COVID-19 that requires hospitalization or intensive care may result in cognitive deficits and other brain damage that are <a href=\"https:\/\/doi.org\/10.21203\/rs.3.rs-3818580\/v1\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:equivalent to 20 years of aging;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">equivalent to 20 years of aging<\/a>.<\/p>\n<\/div>\n<\/li>\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Laboratory experiments in human and mouse brain <a href=\"https:\/\/hsci.harvard.edu\/organoids\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:organoids;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">organoids<\/a> designed to emulate changes in the human brain showed that SARS-CoV-2 infection triggers the <a href=\"https:\/\/doi.org\/10.1126\/sciadv.adg2248\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:fusion of brain cells;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">fusion of brain cells<\/a>. This effectively short-circuits brain electrical activity and compromises function.<\/p>\n<\/div>\n<\/li>\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Autopsy studies of people who had severe COVID-19 but died months later from other causes showed that <a href=\"https:\/\/doi.org\/10.1038\/s41586-022-05542-y\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:the virus was still present in brain tissue;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">the virus was still present in brain tissue<\/a>. This provides evidence that contrary to its name, SARS-CoV-2 is not only a respiratory virus, but it can also enter the brain in some individuals. But whether the persistence of the virus in brain tissue is driving some of the brain problems seen in people who have had COVID-19 is not yet clear.<\/p>\n<\/div>\n<\/li>\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Studies show that even when the virus is mild and exclusively confined to the lungs, it can still provoke inflammation in the brain and <a href=\"https:\/\/doi.org\/10.1016\/j.cell.2022.06.008\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:impair brain cells\u2019 ability to regenerate;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">impair brain cells\u2019 ability to regenerate<\/a>.<\/p>\n<\/div>\n<\/li>\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">COVID-19 can also <a href=\"https:\/\/doi.org\/10.1038\/s41593-024-01576-9\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:disrupt the blood brain barrier;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">disrupt the blood brain barrier<\/a>, the shield that protects the nervous system \u2013 which is the control and command center of our bodies \u2013 making it \u201cleaky.\u201d Studies using imaging to assess the brains of people hospitalized with COVID-19 showed disrupted or leaky blood brain barriers in those who experienced brain fog.<\/p>\n<\/div>\n<\/li>\n<li class=\"ml-4 list-disc\">\n<div class=\"grid grid-cols-[[fullbleed-start_body-start]_auto_[body-end_fullbleed-end]]\">\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">A large preliminary analysis pooling together data from 11 studies encompassing almost 1 million people with COVID-19 and more than 6 million uninfected individuals showed that COVID-19 <a href=\"https:\/\/papers.ssrn.com\/sol3\/papers.cfm?abstract_id=4716751\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:increased the risk of development of new-onset dementia;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">increased the risk of development of new-onset dementia<\/a> in people older than 60 years of age.<\/p>\n<\/div>\n<\/li>\n<\/ul>\n<h2 class=\"col-body mb-4 text-xl font-bold md:text-2xl\">Drops in IQ<\/h2>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Most recently, a new study published in the New England Journal of Medicine <a href=\"https:\/\/doi.org\/10.1056\/NEJMoa2311330\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:assessed cognitive abilities;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">assessed cognitive abilities<\/a> such as memory, planning and spatial reasoning in nearly 113,000 people who had previously had COVID-19. The researchers found that those who had been infected had significant deficits in memory and executive task performance.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">This decline was evident among those infected in the early phase of the pandemic and <a href=\"https:\/\/theconversation.com\/delta-variant-makes-it-even-more-important-to-get-a-covid-19-vaccine-even-if-youve-already-had-the-coronavirus-164203\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:those infected when the delta;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">those infected when the delta<\/a> and <a href=\"https:\/\/theconversation.com\/will-omicron-the-new-coronavirus-variant-of-concern-be-more-contagious-than-delta-a-virus-evolution-expert-explains-what-researchers-know-and-what-they-dont-169020\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:omicron variants;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">omicron variants<\/a> were dominant. These findings show that the risk of cognitive decline did not abate as the pandemic virus evolved from the ancestral strain to omicron.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">In the same study, those who had mild and resolved COVID-19 showed cognitive decline equivalent to a three-point loss of IQ. In comparison, those with unresolved persistent symptoms, such as people with persistent shortness of breath or fatigue, had a six-point loss in IQ. Those who had been admitted to the intensive care unit for COVID-19 had a nine-point loss in IQ. Reinfection with the virus contributed an additional two-point loss in IQ, as compared with no reinfection.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Generally the average IQ is about 100. An IQ above 130 indicates a highly gifted individual, while an IQ below 70 generally indicates a level of intellectual disability that may require significant societal support.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">To put the finding of the New England Journal of Medicine study into perspective, I estimate that a three-point downward shift in IQ would increase the number of U.S. adults with an IQ less than 70 from 4.7 million to 7.5 million \u2013 an increase of 2.8 million adults with a level of cognitive impairment that requires significant societal support.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Another study in the same issue of the New England Journal of Medicine involved more than 100,000 Norwegians between March 2020 and April 2023. It <a href=\"https:\/\/doi.org\/10.1056\/NEJMc2311200\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:documented worse memory function;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">documented worse memory function<\/a> at several time points up to 36 months following a positive SARS-CoV-2 test.<\/p>\n<h2 class=\"col-body mb-4 text-xl font-bold md:text-2xl\">Parsing the implications<\/h2>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Taken together, these studies show that COVID-19 poses a serious risk to brain health, even in mild cases, and the effects are now being revealed at the population level.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">A recent analysis of the <a href=\"https:\/\/www.census.gov\/programs-surveys\/cps.html\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:U.S. Current Population Survey;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">U.S. Current Population Survey<\/a> showed that after the start of the COVID-19 pandemic, an <a href=\"https:\/\/www.nytimes.com\/2023\/11\/13\/upshot\/long-covid-disability.html\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:additional 1 million working-age Americans;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">additional 1 million working-age Americans<\/a> reported having \u201cserious difficulty\u201d remembering, concentrating or making decisions than at any time in the preceding 15 years. Most disconcertingly, this was mostly driven by younger adults between the ages of 18 to 44.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Data from the European Union shows a similar trend \u2013 in 2022, 15% of people in the EU <a href=\"https:\/\/ec.europa.eu\/eurostat\/en\/web\/products-eurostat-news\/w\/ddn-20240129-1\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:reported memory and concentration issues;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">reported memory and concentration issues<\/a>.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Looking ahead, it will be critical to identify who is most at risk. A better understanding is also needed of how these trends might affect the educational attainment of children and young adults and the economic productivity of working-age adults. And the extent to which these shifts will influence the epidemiology of dementia and Alzheimer\u2019s disease is also not clear.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">The growing body of research now confirms that COVID-19 should be considered a virus with a significant impact on the brain. The implications are far-reaching, from individuals experiencing cognitive struggles to the potential impact on populations and the economy.<\/p>\n<p class=\"col-body mb-4 text-lg md:leading-8 break-words min-w-0\">Lifting the fog on the true causes behind these cognitive impairments, including brain fog, will require years if not decades of concerted efforts by researchers across the globe.
