Or should the possibility of neurodevelopment effects be prioritized, even as studies continue?<\/p>\n<p class=\"dcr-1eu361v\">\u201cFluoride is the perfect example of helping people without them even having to do anything,\u201d said Dr Sreenivas Koka, the former dean of the University of Mississippi Medical Center\u2019s school of dentistry. The CDC again <a href=\"https:\/\/www.cdc.gov\/fluoridation\/about\/statement-on-the-evidence-supporting-the-safety-and-effectiveness-of-community-water-fluoridation.html\" data-link-name=\"in body link\">endorsed the practice<\/a> in a scientific statement this May, saying it found no \u201cconvincing scientific evidence linking community water fluoridation with any potential adverse health effect or systemic disorder such as an increased risk for cancer, Down syndrome, heart disease, osteoporosis and bone fracture, immune disorders, low intelligence, renal disorders, Alzheimer disease, or allergic reactions\u201d.<\/p>\n<p class=\"dcr-1eu361v\">Still, controversy over water fluoridation recently made headlines after two high-profile reports and a federal court ruling. The US National Toxicology Program <a href=\"https:\/\/www.theguardian.com\/society\/article\/2024\/aug\/23\/fluoride-water-study\" data-link-name=\"in body link\">set off a firestorm<\/a> in August when it published a <a href=\"https:\/\/ntp.niehs.nih.gov\/sites\/default\/files\/2024-08\/fluoride_final_508.pdf\" data-link-name=\"in body link\">systematic review<\/a> that found with \u201cmoderate confidence\u201d that children exposed to fluoride levels twice those recommended for drinking water (1.5mg per liter versus the recommended 0.7mg per liter) \u201care consistently associated with lower IQ in children\u201d.<\/p>\n<p class=\"dcr-1eu361v\">Then, in October, a new Cochrane Review lowered the estimated impact of fluoride, citing the widespread use of fluoride in toothpaste beginning in 1975.<\/p>\n<p class=\"dcr-1eu361v\">\u201cStudies conducted in 1975 or earlier showed a clear and important effect on prevention of tooth decay in children,\u201d Cochrane Review researchers <a href=\"https:\/\/www.cochrane.org\/CD010856\/ORAL_does-adding-fluoride-water-supplies-prevent-tooth-decay\" data-link-name=\"in body link\">wrote<\/a>. \u201cHowever, due to the increased availability of fluoride in toothpaste since 1975, it is unlikely that we will see this effect in all populations today.\u201d<\/p>\n<p class=\"dcr-1eu361v\">The public debate comes as the National Institutes of Health (NIH), the world\u2019s largest publicly funded research agency, is both a key funder of research advancing society\u2019s understanding of fluoride\u2019s non-dental health effects and an agency squarely in Trump\u2019s <a href=\"https:\/\/www.npr.org\/2024\/11\/12\/nx-s1-5183014\/trump-election-2024-nih-rfk\" data-link-name=\"in body link\">crosshairs<\/a>.<\/p>\n<p class=\"dcr-1eu361v\">The agency is one of the top targets for cuts and restructuring. Paradoxically, that could mean that critics such as Kennedy cut research on fluoride, even as the NIH funds research into potential detriments. Its decay-resistant properties were confirmed in a landmark 1945 study in Grand Rapids, Michigan, which found children in Grand Rapids were <a href=\"https:\/\/www.sciencehistory.org\/stories\/magazine\/pipe-dreams-americas-fluoride-controversy\/\" data-link-name=\"in body link\">60% less likely<\/a> to develop dental caries (better known as cavities) with fluoride added to water.<\/p>\n<p class=\"dcr-1eu361v\">By 1999, the CDC <a href=\"https:\/\/www.cdc.gov\/mmwr\/preview\/mmwrhtml\/00056796.htm\" data-link-name=\"in body link\">hailed<\/a> water fluoridation as one of public health\u2019s greatest victories, alongside seat belts and vaccination. That view was buttressed by a <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26092033\/\" data-link-name=\"in body link\">2015 Cochrane Review<\/a>, considered the gold standard, that found fluoridating water at 0.7mg per liter led to a nearly <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26092033\/\" data-link-name=\"in body link\">26% reduction<\/a> in tooth decay, a figure <a href=\"https:\/\/www.ada.org\/about\/press-releases\/community-water-fluoridation-is-effective-at-preventing-cavities\" data-link-name=\"in body link\">still cited<\/a> by the American Dental Association (ADA) and American Academy of Pediatrics (AAP) today.<\/p>\n<p class=\"dcr-1eu361v\">The need for preventive oral health solutions is profound in the US \u2013 more than <a href=\"https:\/\/www.carequest.org\/about\/press-release\/new-report-685-million-adults-us-dont-have-dental-insurance-may-rise-914\" data-link-name=\"in body link\">68 million people<\/a> lack dental insurance and about <a href=\"https:\/\/www.carequest.org\/system\/files\/CareQuest_Institute_Americans-Are-Still-Not-Getting-Dental-Care-They-Need_3.pdf\" data-link-name=\"in body link\">one in four<\/a> adults said in a survey they have avoided the dentist because of cost.<\/p>\n<p class=\"dcr-1eu361v\">In addition, school children lose an estimated average of <a href=\"https:\/\/www.aapd.org\/globalassets\/naaval-school-days-lost-2018.pdf\" data-link-name=\"in body link\">34m school hours<\/a> each year due to unexpected dental visits; <a href=\"https:\/\/www.theguardian.com\/us-news\/article\/2024\/may\/24\/bernie-sanders-dental-health-care-bill\" data-link-name=\"in body link\">2 million Americans<\/a> visit the emergency room each year because of tooth pain; half a million travel abroad for cheaper care and one in five American seniors lack a single natural tooth. Lack of dental access is so common in some areas, doctors in prison frequently have patients who have never seen one.<\/p>\n<p class=\"dcr-1eu361v\">\u201cOne of the things I routinely ask young people in juvenile jail is: \u2018Do you have a doctor? Do you have a dentist?\u2019\u201d said Dr Fred Rottnek, former medical director of St Louis county jails in Missouri and now a professor of community medicine at Saint Louis University School of Medicine. \u201cA lot of them have never reported going to the dentist.\u201d<\/p>\n<p class=\"dcr-1eu361v\">Modern inquiry into fluoride\u2019s non-dental health effects began to pick up pace in 2015, when the National Toxicology Program (NTP) <a href=\"https:\/\/www.federalregister.gov\/documents\/2015\/11\/23\/2015-29734\/national-toxicology-program-board-of-scientific-counselors-announcement-of-meeting-request-for\" data-link-name=\"in body link\">requested<\/a> a systematic review on fluoride\u2019s impact on neurodevelopment. By 2019, toxicology researcher Bruce Lanphear, of Simon Fraser University in Canada, co-authored a study finding fluoride exposure was <a href=\"https:\/\/jamanetwork.com\/journals\/jamapediatrics\/fullarticle\/2748634\" data-link-name=\"in body link\">associated<\/a> with decreased IQ, which would later be incorporated into the NTP\u2019s systematic review.<\/p>\n<figure id=\"dc4238f8-411a-421e-89ab-be627286fd68\" data-spacefinder-role=\"richLink\" data-spacefinder-type=\"model.dotcomrendering.pageElements.RichLinkBlockElement\" class=\" dcr-1your1i\"><gu-island name=\"RichLinkComponent\" priority=\"feature\" deferuntil=\"idle\" props=\"{&quot;richLinkIndex&quot;:19,&quot;element&quot;:{&quot;_type&quot;:&quot;model.dotcomrendering.pageElements.RichLinkBlockElement&quot;,&quot;prefix&quot;:&quot;Related: &quot;,&quot;text&quot;:&quot;\u2018Your mouth becomes a minefield\u2019: the Americans who can\u2019t afford the dentist&quot;,&quot;elementId&quot;:&quot;dc4238f8-411a-421e-89ab-be627286fd68&quot;,&quot;role&quot;:&quot;richLink&quot;,&quot;url&quot;:&quot;https:\/\/www.theguardian.com\/us-news\/2021\/may\/04\/americans-dental-dentist-teeth-health-insurance&quot;},&quot;ajaxUrl&quot;:&quot;https:\/\/api.nextgen.guardianapps.co.uk&quot;,&quot;format&quot;:{&quot;design&quot;:10,&quot;display&quot;:0,&quot;theme&quot;:0}}\"\/><\/figure>\n<p class=\"dcr-1eu361v\">\u201cThat gives you an indication that the science of fluoride is starting to evolve \u2013 it wasn\u2019t set in stone 70 years ago,\u201d he said.<\/p>\n<p class=\"dcr-1eu361v\">Lanphear, and a small group of like-minded toxicology researchers, argue now is the time for us to \u201cpause and have an independent scientific committee look at all this new evidence\u201d as \u201cwe have a lot of new science specifically about fluoride and the developing brain,\u201d he said.<\/p>\n<p class=\"dcr-1eu361v\">Critics were buttressed again when a <a href=\"https:\/\/www.theguardian.com\/us-news\/2024\/oct\/04\/fluoridation-water-epa-risk-assessement\" data-link-name=\"in body link\">federal court ruled<\/a> in September that the Environmental Protection Agency (EPA) needed to evaluate fluoride under the Toxic Substances Control Act.<\/p>\n<p class=\"dcr-1eu361v\">But physicians, dentists and mainstream professional associations from the American Dental Association to the American Academy of Pediatrics stand by recommendations for fluoridated water.<\/p>\n<p class=\"dcr-1eu361v\">While some towns <a href=\"https:\/\/apnews.com\/article\/fluoride-water-kennedy-cdc-47a0067edb128dbd9b9211a36e674b2c\" data-link-name=\"in body link\">step away<\/a> from fluoride, communities such as <a href=\"https:\/\/www.buffalony.gov\/CivicAlerts.aspx?AID=1430\" data-link-name=\"in body link\">Buffalo, New York<\/a>, are restarting programs.<\/p>\n<p class=\"dcr-1eu361v\">\u201cThis is a shame if we don\u2019t take advantage of what we know from the science,\u201d said Koka about the preventive effects of fluoride. \u201cAre there challenges to doing it right? Yes, but should they be so strong they overcome trying at all? That\u2019s a tragedy.\u201d<\/p>\n<p class=\"dcr-1eu361v\">The CDC has also refined guidance about fluoridation in recent years. In 2015, its water fluoridation recommendations went down to 0.7mg per liter of water, from 0.7 to 1.0mg per liter dependent on climate. Later, the CDC <a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/68\/wr\/mm6804a3.htm\" data-link-name=\"in body link\">issued a 2019 report<\/a> that advised parents of children younger than two to speak with their dentist about fluoridated toothpaste, and reminded parents of children younger than three to use only a \u201cgrain of rice\u201d-sized \u201csmear\u201d.<\/p>\n<p class=\"dcr-1eu361v\">As <a href=\"https:\/\/www.theguardian.com\/us-news\/2024\/oct\/04\/fluoridation-water-epa-risk-assessement\" data-link-name=\"in body link\">critics argue<\/a> that federal agencies\u2019 recommendation lag behind, the US government\u2019s National Institutes of Health (NIH) has become one of the chief funders of research into fluoride\u2019s potential impacts on IQ.<\/p>\n<p class=\"dcr-1eu361v\">\u201cGetting funding from the NIH shows they are interested in this important question,\u201d said Christine Till, an assistant professor at York University in Canada and a co-investigator with Lanphear, whose grant <a href=\"https:\/\/reporter.nih.gov\/search\/HGvfpp16HUm9rzZlWxm5Mw\/project-details\/10848505\" data-link-name=\"in body link\">studying tooth dentin<\/a> and neurodevelopment in Canada was funded by the NIH, but turned down by the Canadian government.<\/p>\n<figure id=\"e6074bb0-c398-475f-8cbb-2fa8d00160b6\" data-spacefinder-role=\"richLink\" data-spacefinder-type=\"model.dotcomrendering.pageElements.RichLinkBlockElement\" class=\" dcr-1your1i\"><gu-island name=\"RichLinkComponent\" priority=\"feature\" deferuntil=\"idle\" props=\"{&quot;richLinkIndex&quot;:29,&quot;element&quot;:{&quot;_type&quot;:&quot;model.dotcomrendering.pageElements.RichLinkBlockElement&quot;,&quot;prefix&quot;:&quot;Related: &quot;,&quot;text&quot;:&quot;Fluoride in drinking water at twice safe limit linked to lower IQ in children&quot;,&quot;elementId&quot;:&quot;e6074bb0-c398-475f-8cbb-2fa8d00160b6&quot;,&quot;role&quot;:&quot;richLink&quot;,&quot;url&quot;:&quot;https:\/\/www.theguardian.com\/society\/article\/2024\/aug\/23\/fluoride-water-study&quot;},&quot;ajaxUrl&quot;:&quot;https:\/\/api.nextgen.guardianapps.co.uk&quot;,&quot;format&quot;:{&quot;design&quot;:10,&quot;display&quot;:0,&quot;theme&quot;:0}}\"\/><\/figure>\n<p class=\"dcr-1eu361v\">Others, such as Ashley Malin, epidemiology professor at University of Florida, and Dana Goin, assistant professor of epidemiology at Columbia University, have also received NIH funding to research fluoride\u2019s non-dental health effects. Malin is studying fluoride\u2019s impact on <a href=\"https:\/\/reporter.nih.gov\/search\/No1mgDI0vE6bfKSYjSxtOQ\/project-details\/10907706\" data-link-name=\"in body link\">children\u2019s sleep<\/a> and Goin is investigating <a href=\"https:\/\/reporter.nih.gov\/search\/_EXx0VmYiEef7lSLuAm_Mw\/project-details\/10907845\" data-link-name=\"in body link\">reproductive health effects<\/a>.<\/p>\n<p class=\"dcr-1eu361v\">\u201cThere is a lot of ongoing work in this area, particularly in the US,\u201d said Goin. \u201cHopefully, the results from these studies will help determine whether EPA drinking water regulations and CDC recommendations for water fluoridation are adequately balancing improvements in dental health from fluoridation versus any potential negative effects.\u201d<\/p>\n<p class=\"dcr-1eu361v\">Goin\u2019s current funding builds on her previous work, which found water fluoridation was not associated with small-for-gestational age or preterm births.
